2027 SPONSORSHIP AGREEMENT

This agreement is between Rooted Seniors, Inc., an Indiana 501(c)(3) nonprofit (EIN 39-4300623) ("Rooted"), and the sponsor named in the form below ("Sponsor"), for January 1 through December 31, 2027. Checking the agreement box and typing your name and title in the form serves as your signature.

PAYMENT TERMS

1. Payments are made through Rooted's secure online payment link. Sponsors who pay the full annual amount by January 15, 2027 receive a 10% discount. Monthly payments are the annual amount divided by 12 and are charged on the 1st of each month.

2. The sponsorship begins once Rooted receives this signed agreement and the first payment. Benefits are paused if a payment is more than 30 days late and resume once the account is current.

3. Sponsorships are non-refundable. If Rooted cancels or reschedules an event, Rooted will provide recognition of equal value at another event or online.

4. Rooted will provide a written acknowledgment of the gift for the Sponsor's tax records. Sponsor is responsible for determining deductibility with its own tax advisor.

MATERIALS & PERMISSIONS

5. Sponsor will provide its logo, headshot, tagline, short bio, website, social handles, and podcast talking points within 10 days of signing.

6. Sponsor grants Rooted permission to use its name, logo, and provided materials to recognize the sponsorship during the term. Rooted writes and records all podcast sponsor messages based on the Sponsor's talking points.

7. Photos, video, and audio recorded at Rooted events and on the podcast may be used by Rooted for education and promotion.

8. Any public advertising by the Sponsor that uses Rooted's name or logo must be approved by Rooted first. Sponsor will correct or remove inaccurate promotions promptly on request.

EVENT GUIDELINES

The Senior Living Series is first and foremost an educational event, not a sales event. All Sponsor representatives agree to:

9. Educate and inform without being "salesy." Sales presentations belong in private appointments outside Rooted events.

10. Arrive at least 30 minutes before the start time and staff the table for the full event.

11. Provide up to 3 handouts (plus business cards) at the Sponsor table only. No chair drops, and no materials on attendee tables or chairs.

12. Display only materials for the Sponsor's own organization, plus educational resources that don't promote non-sponsoring businesses.

13. Not ask attendees for contact information or to fill out sign-in sheets at the Sponsor table. Attendee lists are not shared; when an attendee requests information, Rooted will connect them with the Sponsor.

14. Refer questions outside the Sponsor's expertise to a Rooted team member, and avoid discussing competitors' practices.

15. Wear professional or business-casual attire and collaborate in good faith with other sponsors, panelists, and volunteers.

INDEPENDENCE OF PROGRAMMING

16. Sponsorship does not influence the selection of panelists, podcast guests, or topics. Rooted keeps full editorial control of its events and podcast.

PARTNERSHIP COMMITMENTS

17. Sponsor agrees to make a good-faith effort to share Rooted posts at least monthly, invite clients and families to relevant events, and tell Rooted by the 1st of each month which events it will attend.

GENERAL

18. Rooted may adjust benefits for reasons outside its control (venue, schedule, or platform changes) and will provide benefits of equal value.

19. Either party may end this agreement with 30 days' written notice. Rooted may end it immediately if these guidelines are seriously or repeatedly violated. Payments already made are not refunded.

20. This agreement applies to all Sponsor representatives who participate. The signer confirms that each of them has read and will follow it.
